SOCIAL SERVICES ASSISTANT

This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD Workforce to recruit and appoint qualified candidates to positions in the competitive service. About the Position: This is an open continuous announcement. Multiple vacancies exist and may be filled from this announcement at any of the locations listed. Coordinate care for Soldiers to assist in accessing the appropriate level of mental health care.

Develop support caseload as directed by the clinical mental health providers.

Maintain communication with Soldiers and provider on a weekly basis, or more frequent basis as directed.

Ensure achievement of developed treatment goals through facilitation, negotiation, procurement, advocacy, communication, empowerment, quality care, and involvement with patients.

Maintain connectivity with case managers to assure follow-up plans are carried out.

Utilize automated healthcare systems to input and monitor patient data.

Document all interactions, appropriate issues pertinent to the soldier's case as directed by the Behavioral Health Provider.

Maintain ongoing comprehensive supportive care with identified high-risk soldiers during treatment.
